What is the difference between FS-240 and DS 1350?
The DS-1350 contains the same information as that on the current version of Consular Report of Birth FS-240. The DS-1350 is not issued outside the U.S. (3) A Report of Birth Abroad of a U.S. Citizen (Form FS-240). The Department of State consular office prepares and issues this.
How can I get FS-240 form?
Usually, the local U.S. embassy/consulate website will have this information. After the application is approved, the consular officer will issue a Form FS-240 document, or Consular Report of Birth Abroad. The parent(s) will receive an original copy.
What is a form AE 360?
The AE 360 form is issued by the United States Army as you were more than likely born in an Army Hospital in Germany. This is not the same as the Consular Report of Birth Abroad (FS-240) which is issued by the Department of State.

Is there any problem if there is no surname in passport?

If there is no surname in your passport, stop right here and get your name changes in your passport first before going forward with any application or study abroad process. Empty (or blank) Surname in Passport equals TROUBLE.
What if US visa has no surname on passport?
